Registered Nurses (RNs) and Licensed Practical Nurses (LPNs)

 
At BHC Homecare, our nurses are the clinical backbone of our home care services. We are looking for licensed, dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide elite clinical oversight and help Baltimore-area seniors safely retain independence at home. 
As a BHC Nurse, you will transition from fast-paced hospital burnout to a highly impactful, autonomous role where you focus on one patient at a time. You will directly guide care outcomes, empower family caregivers, and build meaningful relationships within your local community
 
Core Nursing Responsibilities
  • Clinical Oversight: Supervise clinical safety standards and oversee home health teams across the BHC Homecare Service Area.
  • Care Plan Management: Develop customized, nurse-driven care plans tailored entirely to the physical and cognitive needs of each client.
  • Weekly Check-Ins: Perform weekly in-home health evaluations, update clinical care records, and closely monitor patient progress.
  • Transitional Guidance: Coordinate directly with discharge planners and families to assist with smooth hospital-to-home transitions.
